The PTSD-Initial VA-DBQ is a form with restricted access.

This is why you won’t see it listed on the VA-DBQ site where you can view and download all the publicly available forms.

You can only use the PTSD-Initial automated report in Tag if you can demonstrate that you have been granted access to the PTSD-Initial fillable pdf form from the VA.
